We, Cardiac Centre International and its related corporations (as defined in Section 4(1) of the Companies Act), collectively referred to as “us”, “our” or “we”, recognise the importance of the collection, storage, use and disclosure of the Personal Data of persons interacting with us. This Privacy Notice explains our practices.

1. What is Personal Data?

Personal Data is defined in the Personal Data Protection Act 2012 to mean any data, whether true or not, about an individual who can be identified from that data, or from that data and other information to which we have or are likely to have access. This may include name, age, gender, date of birth, telephone number, address, e-mail address and credit card details.

2. Consent for the collection of Personal Data

When you provide your Personal Data to us, you consent by your action to the collection, storage, use and disclosure of your Personal Data by us in accordance with this Notice. If you provide Personal Data relating to a third party (for example, information about your child, dependent, spouse, parent or ward), you represent that the consent of that third party has been obtained.

3. Collection of Personal Data

Personal Data may be collected by us via any form of interaction with you, including: entering into a transaction with us; using our services through our clinic or website; making enquiries, requests, applications and feedback; participating in events, marketing and promotions; through CCTV while you are on our premises; and by applying for a job with us.

4. Refusal to provide Personal Data or withdrawal of consent

All Personal Data is provided voluntarily. You may refuse to provide it, or withdraw your consent, at any time. However, if you limit or do not provide the requested Personal Data, we may be unable to process your enquiry, request or application, or to provide our services to you in a continuous manner.

5. Use of Personal Data

Your Personal Data may be used by us, independently or with our partners, for: identification and verification; processing your transactions; managing the safety and security of our clinic; providing the services you have requested; responding to enquiries, requests and feedback; communicating with you on our services through channels you have approved; internal functions such as audit, reporting and risk management; and complying with applicable law. We may disclose your Personal Data to service providers and agents, in Singapore or overseas, and will use best efforts to require them to protect it.

6. Management of Personal Data

Reasonable security arrangements, including hardware and software technologies, have been put in place to protect your Personal Data against unauthorised access, use, disclosure, copying, modification and disposal. We will not be responsible for relying on inaccurate or incomplete Personal Data provided by you, or arising from your failure to update it.

7. Request for access, corrections and withdrawal of consent

If you have any enquiries, requests or feedback relating to your Personal Data or this Notice , including a request for a copy of your Personal Data, to withdraw consent, or to correct your Personal Data , please email [email protected]. We may verify your identity before fulfilling a request, and changes may take up to 30 days to be reflected.

8. Retention of Personal Data

We have measures in place so that your Personal Data is destroyed or anonymised as soon as it is reasonable to assume that the purpose for which it was collected is no longer served by its retention, and that retention is no longer necessary for legal or business purposes.

9. Miscellaneous

We may update this Notice from time to time. This Notice supplements but does not supersede or replace any other consents you may previously have provided. It is construed and governed under the laws of Singapore.
